NEW PATIENT FORMS:
Aspen Counseling and Recovery Services, PLLC is a confidential counseling service. Aspen Counseling is held by State and Federal laws of confidentiality of both mental health and substance abuse services. Once an appointment is made, no information can be disclosed to anyone without your written permission on a Release of Information form. When you come to your first appointment, the policy on confidentiality and your rights as a client is reviewed in depth. If you have any questions concerning confidentiality before our first appointment, feel free to ask them in the initial phone interview.
